Please bring to your first appointment:

 A list of your current medications, with dose and frequency

 Your doctor’s referral/order form

 Your current insurance card

 Insurance authorization, if applicable

 A photo ID

Please fill out a registration form, and bring that with you

If you’ll be visiting us for an amniocentesis, CVS, or genetic counseling,
please complete the summary of your genetic counseling questionnaire,
